As pictured below, many of my pages for local cities have my location listed in the results in blue.

Not all, but many.

Why is it, how is it, why only some, not all?

its cause the address is there on site...and matches other NAP across the web........

as to why not in all areas....my guess is because of the actual proximity to the query ....Mill Creek being closer to Snohomish than woodinville

the answer to your next question is he shows the address because the quality of his site outweighs the weight given to proximity..............otherwise only Redmond/woodinville addresses would be displayed for the Woodinville query

Google does what it wants, when it wants , to who it wants! I had some of our reviews put on a competitors page, listing drop, 1st page organic listing drop to 30th for no reason at all? It's called the google dance, and it will go on forever if that's your form of gaining new clients. I don't like to dance!
