Okay, Here's what I'm asking them for:

Name:
Address
City state zip
Phone: Day Eve
Email Address

I am interested in:
Carpet Cleaning
Upholstery Cleaning
Area Rug Cleaning
Deodorization
Black Light Inspection

What other information should I put on there. I have a box that is almost 1 quarter of the card where I can put more information

Should I have fine print about the contest, should I have a checklist of the best way to contact them? Should I have a spot where it says "I would like to recieve special offers by email?"

I like to include; "Entire form must be filled out to qualify for drawing"

Instead of "I'm interested in..... "Id put "I'm interested in free quote for. "(Make check boxes for the selection.)

I also put on the entry slip " My house was last professional cleaning on;(Then check boxes reading); 6 mo. 1year. 2 years, Too long."

The whole idea is to determine the interest they may have in your services.

Tailor your questions so that you can qualify them for future mailings or other follow up.

If you have MS Publisher I'll send you the follow up card I send out after the drawing.

It basically says that they won the 2nd place prize.....

I have print shop 22... If you save it as a PDF I would at least like to look at it.
