Commercial Route Sheet

Does anyone have a "commercial route sheet" where have columns for each business, business type, date, etc...I'm having my commercial sales rep go after accounts and I have noted down the areas/businesses I want her to go after, would be easier to do this way for both her and I.

Yes, mine is in Excel, it works for me although I would like something on my iPad.

Being somewhat computer illiterate, this is not easy.
