Scope of work sheets

I am needing some help please. Our scope of work sheets are in desperate need of updating.
i need a form that will show in detail what work is being done ( extraction, demo, etc) and makes sense.
Anyone mind emailing me a copy of the one they use to gcole@donerestorationinc.com ?

If you are talking about the same document ours isn't all that great either. Just something throw together in publisher but maybe it will give you an idea of what you want????

Thanks. I'm trying to get several samples of what others use and come up with an idiot proof one that will make the estimators life easier

Ours has been ever changing and has morphed from what i once designed to this. The guys add and delete what they need or don't, I no longer deal with it.
