Transitions coming apart

I see a lot of this, especially in rentals. I assume it's because the installer pinched the metal down to tight and weaken the backing. Am I right? Can you suggest different ways of fixing this. What is the proper way to install these transitions to begin with? How tight?

Shane,

This is very common. It has several components to the problem, probably the common denominator has to do with there is usually a tackless strip in front of the metal which causes the backing to break down rather quickly with traffic. Regardless of why, these are things we should just fix and be happy someone did not do their job correctly, it is what I call job security.

To fix it, simply re-laminate it and stretch it back in.
