Bad rotors or too much heat build up due to insufficient break system for the weight your hauling....are you running through pads much faster?
I'd run the van over a scale just to see how much weight you have that should tell you alot right there . if you are under the GVWR then i'd think it's bad rotors and slotted rotors should be the fix at or over GVWR then i would think your break system isn't sufficient to handle the weight.
If that's the case you need a larger break system rotors and calipers.....GM perhaps or after market $$$.
or you could Jerri-rig it and hope the breaks don't fail and kill people or yourself it would probably take a few hours for the fire department to remove the TM from your colon :shock:
